Clinical Efficacy Bricanyl Turbuhaler Treatment in Patient with Bronchial Astham

Dennis Wee
Abstract
Inhalation of fl-agonists is well established in the treatment of asthma and has advantages such as rapid onset of action, good efficacy at a low dose and few side effects. However, many patients are unable to use a pressurized inhaler efficiently despite adequate tuition. The new inhaler, turbuhaler, was developed to facilitate inhalation therapy in asthmatic patients who have difficulties with the correct use of a pressurized aerosol(MDI, metered dose inhaler). The major advantages of turbuhaler are that it circumvents the problem with coordination of actuation and inhalation, and that it contains active medicine without any propellants or additives.
The aim of this study was to investigate the efficacy and the acceptability of Bricayl turbuhaler in the treatment of bronchial asthma.
Twenty patients with mildly and moderately severe asthma were treated with 0.5 mg terbutaline sulphate delivered via a new multi-dose powder inhaler(Turbuhaler) for 4-week period. The effect and acceptability of treatment were determined by twice daily measurement of peak expiratory flow rate(PEFR) and by diary cards (severity score) and questionnaires.
Mean PEFR increased from 343¡¾109(Umin) to 365 ¡¾131(Umin) in the morning, and from 365¡¾ 115(L/min) to 378¡¾131(L/min) before bedtime during 4-week therapy period with Bricanyl turbuhaer. The mean severity score of symptom decreased from 2.65 to 1.95 in daytime, and from 2.2 to 1.4 in nighttime during that 4-week period. This treatment produced a significant improvement in symptoms (70%, 14/20) and in lung function(60%, 12/20). No side effects were reported when the turbuhaler was used. All patients answered that Bricanyl turbuhaler was easy to handle and inhale.
Bricanyl turbuhaler seems to be a valuable alternative to bronchodilator MDI therapy in the treatment of asthma. This device is effective clinically, and has a high degree of patient¢¥s acceptabi-lity.
